Enrollment Forms

For Current In-District or Approved *Inter-District Transfer Students

*If your student resides outside of the TTSD boundaries and has not yet been approved to transfer into the district, please complete the Inter-District Transfer process request to attend TTSD to enroll your student in TTVA upon approval.

If you do not have access to a printer, paper copies can be picked up at the Tigard-Tualatin Virtual Academy office.

Please print and complete the following forms:

• Transfer of Student Records 
• Student Enrollment - Spanish Form
• Oregon Certificate of Immunization 
• Oregon Language Use Survey - Spanish Form
• Oregon Family Income Form 
• ODE Remote testing form - Spanish Form
• Student Contact Update Form (if you wish to make changes after enrollment) - Spanish Form
  
  

Additional information for proper student placement:

• Transcript or grades
• Copy of IEP, 504 etc. for special education (if applicable)
• 23-24 Forecasting Form (complete w/student)

Please provide the following documents upon enrollment:
Birth Certificate or Passport
Complete Immunization Record
Proof of Address (ie. utility bill-water/electricity, rental agreement, mortgage statement)
Power of Attorney (if not legal guardian/custodial parent)
